Try to avoid staying in Medan overnight before getting a bus to Lake Toba.
When I have visited Lake Toba, I like to stay at the Carolina hotel but it has to be booked early. Little on the island but at least it is cooler than the mainland.
I take a bus from there (booked a day or two before. Air con bus is worth every extra rupiah), stopping at a few places like Padang on the way, ferry across to Java and a train to Jakata and then stopping at a few places like Yogyakarta, onto the Bali ferry which connects with a bus the other side, which takes you to Denpasar.
Watch out for pickpockets in Bali. Lots about. And don't get short changed when changing money up either. The mosquitoes come out in serious numbers in the evening.
When you visit Bali, either Ubud and Seminyak are great spots to visit. Awesome nightlife, fine restaurants and shopping stores are located in Seminyak, as well as the beach (Kuta). It's a little bit crowded during peak-season.
Ubud is quieter, and mostly visited by tourists for them to have a serene getaway. It has great beaches and resorts too, and some extraordinary destinations such as museums, temples and restaurants.
If you have any chance, stop by Vietnam.

I think you should fly from KL to Medan. Bukit Lawang and Tangkahan and Danau Toba are surely something you shouldn't miss. I'd suggest you to go to Nias than Mentawai. The ferry schedule to Mentawai is once a week - better go to West Sumatra and stay at Bukittinggi or Sikuai island
