I don't think this fits your menu at all, but the Cinque Terre area is a delight all the same, even if it is full of tourists. I was there on an Italian Bank Holiday & there were swarms of Italians enjoying this marvelous stretch of their coastline.

I will travel to Italy in one week. I was looking for the same..I found PIedmont to be that place.....you should check into this.
Malcesine in Lake Garda is stunning! We went for my brothers wedding last July and it pretty much has all of the above. I would highly recommend it, it is absolutely beautiful.
In june school end in Italy so may be you can find some italian tourist in the place i am going to suggest.
i can suggest you Catania or its surroinding (acitrezza, acicastello, ecc). Beutifull sea, really good food and wine, Etna as vulcuna is a mountain, tourist are almost local. But you can look to some other place in sicily.
I don't know how much bicycle friendly they are.
another place could be maremma (a region) in place like orbetello or manciano or similar, people are for sure bicycle friendly and you can find same again: good food, good wine, hills, sea etc.
if you avoid international tourist (people from de, hl, us, uk, etc) avoid place as: 5 terre, lake garda, rimini are they are really beutyfull place but really full of not italian. In other palce you will find more italian.
1. in northern italy you won't find this easily
2. have you heard many people visiting apulia or sicily? maggiority of tourists go to venice, florence, roma, milan...South is undiscovered so far
3. Again, South, in north they would rather drink spritz, beer, cocktails...regarding cars honking, well that would be a problem as in South we use to honk a lot, even to greet to eachothers
4. there are many towns and cities like that, the only problem is the language, not many italians can speak english but with a bit of effort you can manage to understand
5. Than again, South italy
6. In South there is organized crime, as a fact, but hospitality is still a VALUE and you don't have to be worried at all during your stay (if you behave well, of course)

I have been living in Italy for 13 years now, so I know the country well. Most coastal places get busy from mid-June onwards, with the school holidays etc. However, I would like to recommend 2 places: Procida, which is a tiny island off the coast of Naples (not far from Capri), but a staying in Procida (population approx. 10 000) would not break the bank as Capri would. A beautiful, beautiful place, simple people, mostly fishermen and their families, laundry hanging on washing lines across narrow alley ways, great restaurants and bars (cafés). Locals are friendly and helpful.
The second place I strongly recommend is Scicli in south-east Sicily (population approx. 26 000. A beautiful town with Sicilian baroque architecture everywhere. A bicycle ride away the coastal town of Donnalucata, beautiful beaches, never crowded, crystal clear water, even in the peak summer season (July.August). Nearby other beautiful culturally rich towns, like Ragusa, to mention just one.
