Have you at least contacted the respective Embassies and researched what visas, etc. you require at the land border crossings?

You do not state your country, but most western passport holders will get a free 14 entry permit to Thailand when you arrive overland. If you plan on more than 14 days in Tland, I would either buy a 30 day visa while in Malaysia or fly. You will get a free 30 day entry permit when arriving via air. Decide what is your best option.
After Tland, I would go into Laos via Friendship Bridge. You will get a 30 day visa on arrival, $35 for most passport holders. After Laos, I would go overland into VN. You will have to buy your VN visa while in Vientiane. Allow a few days for this visa. Once in VN, I would use the bus system to travel south to HCMC. Or use system. The bus is cheaper & easier to use, rail is safer. covers my bus trip from HCMC to PP. in if interested in some swimming, snorkeling or diving. to Siem Reap for touring . One long day overland back to BKK. Read about it if you go overland. Or fly r. Expensive, but you are back in BKK in one hour instead of all day on cars & buses. You will get another free 14 day entry permit into Tland, or 30 days if you fly back to BKK.
I suggest this route instead of yours because you won't have to backtrack very much. Good luck with your planning.

Not been to Laos but did the rest by bus/coach/train it's not a problem.
You sometimes have to wait for the buses and they aren't always on time but it's easy.
Buy the bus tickets from a travel agent or from the station probably a couple of days before you want to travel.
We used the sleeper coaches in Vietnam Malaysia Cambodia and coaches and trains in Thailand.
Some Visas you can purchase on the border it all depends on your Nationality and you should check this out with the Embassies before you leave.
Dependent on where you are traveling your might need Malaria tablets, and other jabs ask a healthcare professional, don't take advice from any website on health issues.
I have traveled to these sites, with trips broken up into separate trips. Beware if you are trying to make a schedule, that buses often take longer than they say they will, things happen along the way (one of my bus trips took twice as long as expected because of a landslide in Laos blocking the road!).
Also note, depending on country you may have to turn in your passport to an embassy to get a Visa. I had to drop mine off at the Vietnam Embassy for a whole week while getting approved, so make so to factor that time into your trip!
And yes, check about medications you might need on the trip before you go.

You have to apply for the Vietnam Visa in advance - it's not an 'on arrival' type. You can apply in Cambodia and it takes a couple of days to process (it's also cheaper to do it there).
