It depends what you want it to do.
Personally I forget about any baggage cover or delayed departure payouts - those things are minor and get soaked up by excess charges. I want medical care and repatriation, and pretty much all policies do those properly. So I buy the cheapest, as long as it covers any countries i may go to.
I often take worldwide excl usa & Canada since that is cheaper and I seldom go to them. I look at restrictions eg a max altitude on hiking, and I ensure it covers cruises.
Even so I occasionally end up also taking a single trip policy for longer trips which wouldn't be covered by an annual policy - watch out for that.
