One place I would recommend which isn't on your list is Pai. It's a lovely, chilled out place to spend a few days. We did a good day trek into the hills from there.

Just out of interest, have you been to any of the more remote places that I mentioned above? It's hard to get a grasp of how accessible they are!

No, haven't been to any of the more remote places. I've got a friend who's going to Mae Hong Son in November and it sounds great!
i had planned on going up to Mae Hong Son, but had such a great time in Chiang Mai and Pai, i ran out of time. to get to Mae Hong Son you can go right through Pai. I rented a motorbike in Chiang mai and rode up there, very easy to do, and Chiang Mai has a lot of ex pat owned rental places that can give you advice. you can leave your main bags at your guest house in Chiang mai and then travel light up into the mountains. i also did a mountain bike ride with a ex pat company in Chiang Mai, quite an experience and a great way to meet locals in remote areas, they go a bit crazy when they see a westerner on a bicycle!
