I don't know if motorcycle tours are up your street but I would definitely suggest a dirt bike tour if you want to see the real Cambodia. I recently moved to Cambodia and wanted to see as much of the country as I could so i booked on a 2 week tour with -snip-and it was one of the best things I've ever done! As well as seeing the famous parts (temples at Angkor Wat, the killing fields etc) you get to see the real Cambodia too, going through rural villages and meeting the locals.
Although I'd never done any off-road riding before in my life, the guide and mechanics were so encouraging and helpful I soon picked it up.
I know -snip- tailor make tours so you would be able to pick out the places you want to see and the time scale. I honestly can't recommend it enough.

Book an AirAsia flight to Phnom Penh and then you can get a bus to Siem Reap when ready.
A number of travel agents about who will be glad to help you on tours. The same in Bangkok though that is very easy to do yourself.
I would leave out KL as the other destinations will take all your very limited time.
