Visas?

There's definitely no such thing as a "combined visa". These are all separate countries (assuming you meant "Colombia" rather than Columbia (a US city), and except for "South America", which is a continent), so each have their own visa rules.

For which of these countries you need a visa depends on your nationality, which you unfortunately didn't include, so it's very hard to help you there.

Generally you don't need a visa for a stopover where you don't leave the airport, but there's exceptions (like the USA), so it's worth researching those specific cases, too.

Both Colombia and Costa Rica allow EU citizens to visit for tourist purposes without a visa, as long as you stay for less than 90 days. For the USA you indeed need to go through the ESTA hassle.

for esta you need to apply about 2 weeks before you travel just to be sure you get it
