Sounds very strange to me as I understand the visa to be only to say that you might be allowed into the USA, whereas the entry stamp in your passport is what allows you to stay in the USA.
Apart from your Italy question I would be wondering more what your right of re-entry to the US is & I would be checking that.
As to the Italy question - How about you land in some other country before Italy so you could try asking for a Schengen from that country's Embassy ( even if it is not your main destination as required by the rules) or make another country your main destination.
I would think you just need to get an updated stamp in your passport.
It would be similar to not getting a visa because your passport is about to expire.
Ditto Peter. Update the stamp and it's a non issue.

Thanks all. The problem is I would need to leave the country to get an updated US visa stamp, and given that my trip to Italy is supposed to be in October and the Italian embassy can take up to 21 days to process Schengen visas, I don't think I have enough time.
Are you sure you need to leave the country? Double check that...

I'm quite sure -- I think the idea is that you only need a visa stamp to get into the US, so you (normally) wouldn't need to get an updated US visa stamp once you're already in the US.
