Just to be clear, it's a visa for the Netherlands, not for Amsterdam. You should use the names that are in your documents and your passport above all else. I would assume that's the same as your 'official' name, but if not, I'd still use the passport name as that is what everyone will be checking your paperwork against. Then again, isn't it kind of strange not to have your official name in your passport?

Based on the thread subject, I think there's a "no" missing from SaloniS original sentence. As in, it's supposed to say, "I have no last name in my official name." And so SaloniS' official name (which is of course mirrored in the passport) is just a single name.
I imagine that web forms at least will prevent an obstacle, as those'd typically not be set up to deal with such an exception, and would blindly require both a first name and a last name to be filled in. Hopefully you should be able to apply for a visa just fine if you can get a human to look at things.
I'm curious: WHY don't you have a last name? Even orphans are usually given a last name.
If that's your full name - and that's how it's written on your passport, then it won't be a problem....Simple.

As far as single name is concerned, it is common in the area I live to have only single name as the full name.
It is only in the company I work in, that my father's surname has been appended. However, getting it changed in the company will not be a problem....
