You have several options.
1. Buy a 30 day visa while in Vientiane.
2. Extend your 15 day visa for 7 days, 1,900 baht.
3. Pay the 500 baht/day fine when you hit the border. I recommend you avoid this option. You DO NOT want to be in an overstay situation when in Thailand. The Thais take overstay very seriously, so should you. Here is an on overstaying that you should read. Good luck.

No. You will have to go to Immigration BEFORE your 15 days are up. Fill out the form, photos, 1,900 baht.
