For Bangkok, it depends on what you wanna do in Bkk and how long will you want to stay there. As I saw from your list, when you are in Bkk you probably go to nearby province such as Karnchanaburi where you can do some of these thing:Floating Market, visit to Bridge over the river kwae and war cemetery of allied prisoners. Tiger Temple, walk with Tigers and photos with adult Tigers…Elephant Camp and elephant ride, Feed Wild Monkeys.... And many well known waterfall in Karnchanaburi. [If you really interest in touching the tiger, Tiger temple is really interesting]
By the way for original and the most famous floating market in Thailand is DAMNERNSADUAK FLOATING MARKET which located in Rajburi and not that far from Bkk. You may find some tour programme while you are in Bkk, there are so many type of programme or destination. Here in Rajburi you can enjoy long tail boat to travel along the canal and try any local food or farm or any old fashioned routine of Thais
Or you may go to Pattaya which is the famous destination for tourist and better for travelling with children somehow... You can go to floating market, "Suan nongnuch" where you can enjoy with many type of garden and also have some activities and places for kid too! There are many shows that you may like it such as elephant show, traditional Thai arts and dance and many mores! For cooking class and temple you can find in every city along Thailand
The most wonderful temple that is very well known is in BKK, Grand palace [I think it is one in the list that you'd better visit] Wat Pho, Wat arun , etc. And BKK is full of shopping and dining experience as well.
In my opinion: you may enjoy city life in BKK for 2 nights is enough and Find the tour to nearby provinces which take time about 2 or 3 days tour. Anyway it depends on you that if you wanna travel as chill as you can you may extend the time staying in BKK and nearby city.
Then you fly to the north of Thailand. You can visit many temple in different style from Bkk because most of Temple in Northern Thailand are from different root of culture which means you can find different style of arts and architect. There are many tourist places as well such as PHRATHAT DOI SUTHEP[http://www.doisuthep.com] , etc. [better go here : http://www.chiangmai-guideline.com/]
For staying in Chiangmai, I think 4 or 5 days is enough. Or 3 days still enough but for me I prefer 4 or 5 days because the surroundings there is very nice and easy.
Then you fly to Phuket and have a wonderful time relaxing there with beautiful beaches....
There are so many many things do here and I wish you would happy and get the wonderful moment in Thailand. I may not able to suggest you all of the things you can do here because it's too much! You'd better find more information from travel site or official site of the place....
