In a week you could cover Chitwan National Park, Pokhara and Kathmandu, with some day trips from Kathmandu to surrounding towns. I did an organised tour so I'm not sure about getting around, but I know buses don't have a good reputation out there! I'd definitely recommend getting up early in Pokhara to see the sunrise in Sarangkot.

If you are not hiking than Chitwan, pokhara and Kathmandu will be better and relaxed.
Jungle safari, sunrise, boating and UNESCO world heritage sites will be a good compact for you.

22 sep 2012 Day 02. Half Day Sightseeing in Kathmandu & Flight to Chit wan National Park overnight resort Chitwan
23 sep 2012 Day 03. Chit wan National Park overnight resort chitwan
24 Sep 2012 Day 04. Drive to Pokhara & Tour in Pokhara City
25 sep 2012 Day 05. Tour of Sarankot Hill & Pokhara City, Flight to Kathmandu & Drive to Nagarkot.
26 sep 2012 Day 06. Nagarkot- Tour of Patan City, Bhaktapur Drive back to Kathmandu.
28 sep 2012 Day 07. Tour to Boudhanath, Soyambu Overnight Kathmandu.
29 Sep 2012 Day 08. Departure to International airport. Fly back to Home.

Day 02. Full Day Sightseeing in Kathmandu.
Day 03. Drive to Trisuli River fro Rafting & after rafting drive to Chit wan National Park
Day 04. Chit wan National park
Day 05. Drive to Pokhara & Tour in Pokhara City
Day 06. Tour of Sarankot Hill & Pokhara City, Flight to Kathmandu & Drive to Nagarkot
Day 07. Nagarkot- Tour of Patan City, Bhaktapur Drive back to Kathmandu.
