Definitely bring your warm clothes. The temps can be in the 40s and you travel get to the 10s, so be prepared.

London -check
I could do a day less in Paris, but it depends what you want to see. A nice day trip would be Versailles or Giverny. 4 in Paris + 1 outside?!
Amsterdam - check. I spent 5 days the 1st time I went, again it depends on what you do, but this is plenty of time. 3 days may be average. Last time I was there I spent 2 nights.
Germany: 5 days in Berlin is relaxing, and you will have time to visit Aushwitz, if you want. I've never been to Munich, but being that is in Bavaria, I would spend 4 days (at least for the beer!).
You literary is also kind of jumpy, so maybe after Berlin you head to Prague (4 days), then Vienna (4 days here is plenty, you could even take a ferry to Bratislava, and see a castle), Munich, then Zurich. Between Vienna and Munich, you could stop in Salzburg.
If you decide to travel to Switzerland, I would do so before Italy, and have a mulit-city RT ticket.
At the time your going, I would say Zurich, easily, and 4 days of skiing.
From here, I would head to Milan/Florence for a few days, then Venice, Rome and Naples. The Vatican is half-day in Rome if you go early in the morning. You can see Rome in about 3 days.
Costs can be personal. Where do you want to stay a hostel or hotel? Will you cook your own food, or eat out all the time? Consider everything when you create a budget. You can add up the days you plan on traveling (including flights) and set a daily budget. You can also set a more lenient weekly budget. I would based on experience recommend $1000 a week for a flexible budget.

I forgot to add but we would be looking at budget-style accommodation so hostels or anything that is clean, safe and cheap!
