In my opinion, using your debit/ATM card is the best. There are ATMs all around the world now that accept Cirrus and PLUS. No matter if you chose to use an ATM card or pre-paid card, just make sure you have a credit card handy for the unexpected and flexibility.

I've been looking at the Caxton card too as it does seem one of the best ones out there for fees abroad etc.
Might be worth looking on moneysavingexpert. co.uk, I had a look and theres loads of advice on the best cards to take abroad travelling.
I liked the idea of the Caxton because it seems you only pay a fixed rate exchange fee, no others. Still debating tho lol!
