UK carrier 'Easyjet' does direct non-stop flights in both directions (BARCELONA-MILAN-BARCELONA) for under €30 single.
If you fancy a little adventure and your budget allows it, then why not do Milan-Cluj-(Romania/Transylvania)-Barcelona with 'Wizzair'. A lot of wonderful sightseeing along the way and the fares aren't at all expensive.
Easyjet and Vueling do flights between Milan Malpensa and Barcelona (so not Girona, not Reus). If you want to check for other destinations in Italy, use Skyscanner to do your flight searches.

I've found some information about nearest Barcelona airports. From Milan is possible to fly to Girona with Ryanair, and I think is not so far from Barcelona (http://www.barcelona-girona-airport.com/), to Lleida you can fly with Vueling and it doesn't look so far from there neither (http://www.lleida-airport.es/).
Vuelig has a direct flight to Barcelona and is not too expensive, perhaps is the best option.
The adventure from flyingbob looks intersting, but I think is too much for my economy.
