I'm one of Jakarta's residents.
For those plan, you have enough time but I have to tell you that most of public transportation in my country isn't built very well (yet).
Krakatau island: you need to go to Anyer/Carita beach area & catch a boat there. It may mean that you need to rent the whole boat actually.
Pangandaran: take a bus direction to Tasikmalaya then continue with smaller car. Otherwise, you rent a car + driver somewhere in Jakarta for the whole trip.
Borobudur - Mt Merapi & Jogjakarta: either bus, train or flight from JKT to Jogjakarta. Then see a local travel agent for an arranged tour or other options available.
Bromo & Kawah Ijen: Firstly, go to Malang or Surabaya as the nearest base. Either by bus, train or flight from JKT. There are tour options for Bromo, not sure for Ijen crater. If you want to go non-tour, for Bromo you have to be in Probolinggo then catch a minibus for public that will bring you closer to Bromo mountain.
I'm not very sure of the road's condition either for Bromo or MErapi, because since the explosion happened, they had been closed for tourists. Please check with local authorities for your own safety.
