The slowboat from Northern Thailand to Luang Prabang is great; it takes 2 days and you'll stop overnight in a jungle town called Pak Beng. If you book a 'package' in Pai or Chiang Mai it will normally include packed lunch and a night's accommodation in Chiang Khong on the Thai border. I can't remember the cost but it certainly didn't break our budget and we were travelling on the cheap.

I'd recommend flying from Vientiane to Hanoi; we flew with Vietnam Airlines which took an hour and cost around $100. As far as I know there are no trains and as you say, the bus is infamous as it's a long journey with frequent breakdowns on bad roads. I believe it can take 18 hours!

It's easy to get around Cambodia by bus. You can go across the border from Ho Chi Minh right into Phnom Penh and from there catch various buses around the country and on to Thailand. Which island do you mean?
