To travel from Bangkok to Malaysia I think you will have to go by bus but check the Thai rail timetable.
If your not visiting any places between HCMC and Hanoi then I would fly in to HCMC - travel by bus south and then cross in to Cambodia - consider Kep, Kampot, Sihanoukville - then head north to Phnom Penh - Siem Reap - cross border in to Laos - travel north - cross border at Huay Xai in to northern Thailand - overnight train to Bangkok.

In Cambodia and Laos work on travelling by bus. It will be lot easier to travel south-north/north-south. Most people will fly out of Hanoi although there is a 24hr bus trip that goes to either Vientiane or Luang Prahbang. Many negative reviews on this bus trip though.

You can travel north from Chiang Mai to the Burmese border. I've been to the border on a day trip out of CM. Many blogs state Burma is interesting to visit as long as your out in the country where the people have less contact with the military regime.

If you enter Thailand by air you get a free 30 day visa. If you enter by land you get a free 14/15 day visa. Given your itinerary I think you will need to consider this. Visa for Cambodia and Laos are not free. Good luck with your planning. Google is your friend.
