i've read a few opinions on the southern province of thailand near malaysia being a bit unsettling but also that it's a common border run spot so it's a bit of a contradictory.
As you have read, there has been violence in South Thailand for many years. Who knows what will be going on in Sept? Check news accounts before your trip. It looks like a bus from Sungai Kolok to Kota Bharu, then on to Kuala Besut is your best option for getting to the Perhentian Islands. Good luck.
