Sounds like a good plan and with the time you have I think it is possible. To save some time you might consider flying to Chiang Mai and from Chiang Mai down south. Air Asia often has good deals on flights to Chianbg Mai, that might save you some money.
To get from Chiang Mai down South you can fly directly to Koh Samui with Bangkok Airways. If you're on a budget you can also fly to Surat Thani with Air Asia and take a ferry to the Koh Samui. It takes a bit more time, but the ferry is also a nice experience.

Meeting a tribe and elephant riding can be done in a one day tour in Chiang Mai.
The Nightlife can be found on Koh Samui or Phuket. If you just want to enjoy some nice beaches I would spent my time in Samui and Phangan and skip Phuket.

You can get most of your list done in Chiang Mai (CM) or Chiang Rai (CR). Do some research on. Fewer people in CR so perhaps a . CM has as well. Strolling the CM is almost required!
I did a quick search and only one out of 12 daily flights on is non stop. $250 one way: CNX-USM. The rest go to BKK first. Of course the cheapest option is overnight bus to BKK, spend the day sightseeing in BKK and then overnight bus to Surat Thani, ferry to Samui. Or CNX-BKK, then BKK-URT, ferry over to Samui.
I agree with the above post. Forget Phuket, you will have enough beach with Samui & Phangan. Good luck.
Bangkok - Chiang Mai/Chiang Rai - and Phuket/Samui/Phang Ngan are very dynamic.
Bkk u get night life, culture, temples, food, shopping.
CM/CR u get non-metropolitan culture with the elephants n' all
Phuket/samui/phang ngan - pretty much beach/party areas - all 3 places are pretty expensive - especially samui and phang ngan bcos they are not attached to the mainland and everything sold on the island is more expensive.
Phuket is actually nice if you stay in secluded areas/resorts - unless you want nightlife - which means Karon beach or Patong beach areas.
Try searching online for places to go as well - from thai forums or thai facebook fanpages about traveling.
I have a useful one i use - it's a fanpage on facebook that ppl post pictures of traveling around thailand with chang beer - pretty interesting and fun - search "live like you mean it" on facebook or "chang beer"
