I am planning to go to Cambodia and Thailand for just under 3 weeks and I am wondering the best way to get to all the places I want to go.

Normally I would say by train, but recently Thai State RR has had entirely too many derailments

I would look at the Air Asia and NOK Air websites

As Daawgon has said, check Air Asia and NOK for flights to see if you can get direct flights between the places you want to go.

I much prefer Koh Phi Phi over Phuket. Phuket is too touristy and Phi Phi has a nice party vibe for the younger crowd.

You can get to Phi Phi by ferry from either Phuket or Krabi. Just find a booking agent when you are ready to go and they have buses to shuttle you to the ferry terminal.

Personally I wouldnt want to squeeze that much in 3 weeks but I dont like to rush my trips. You could skip Phi Phi and do Koh Samui instead. It's easy to get a boat from Samui to Koh Phangyan. Koh Phangyan has a nice party vibe as well.
Samui is way nicer than Phuket and you would probably have to do Phuket if you are going to Phi Phi.

The VIP buses in Thailand are great if you want to avoid flying. They are quite comfortable and you could do Bangkok to Koh Samui over night. In the morning, you take the ferry over to the island from the bus terminal.

If you decide to do Phuket and Phi Phi, then you can get cheap flights from Phuket to Chiang Mai. Maybe not so cheap from Samui to Chiang Mai and I'm not sure if there are direct flights.

SIhanoukville you can get to from Phnom Penh quite easy. You could either get a bus or you could share a car. To find a share car, just ask your hotel reception or look for a booking agent. I usually spend extra to get the front seat to myself. They will squeeze as many people into the car as possible, even the driver shares a seat.

www.skyscanner.net is a good way to check what airlines will do what routes. There might be some small airline that will do that route but I would doubt it.

You should check out cambodia angkor air. Brand new planes, super service etc. and if you're lucky they might have some super saver deals. Its a relative new airline run by Vietnam Air.

Went to Cambodia last year 4 weeks and flew from SR to Snookyville and PP to Bangkok, both with Cambodia Angkor Air. So I can vouch for them.
SR to snooky was a new ATR-72 turbo prop, 88 USD, and PP to Bangkok new Airbus320, 96 USD.
Both trips around 1 hour.

Busses come in all sizes and shapes. We went from Snooky to kampot with taxi, 3 hours, and Kampot to PP with VIP bus(service, big screen tv, free wifi) approx. 5 hours- 12 USD.

Just ask at the desk where you are staying since they know the local transport lines better and can arrange everything for you.
