Hi Pali,
If you want to travel from Phnom Pehn to Ha Tien, you can catch a bus to get Kampot Province. From there, you can cross to Ha Tien and take a speed boat to Phu Quoc island. You should spend 2 nights/3 days on the island. When you come back, chose to arrive to Rach Gia city (Phu Quoc - Rach Gia). from there, you can join a tour to visit U Minh Thuong National Park. Just google it you will see some information about the park. then you can back to Ho Chi Minh later.

Can Tho is the most developing city in Mekong Delta so it is better if you have one night in Can Tho city. You can explore Ninh Kieu at night. In the early next morning you can visit Cai Rang floating market. Then come back Ho Chi Minh city.

I had a look at your plan, some information will hopefully be helpful for you:
- Travel from Phnom Penh to Ha Tien
-“Take a speed boat to Phu Quoc. Spend x2 days on the island”. At phu quoc: you can visit Tranh stream, Cau temple, Phu Quoc prison, Ham Ninh fishing village, relax at the Sao beach, fishing, kayaking, water-moto, parasailing at An thoi port, ect. Hotels & resorts: good, reasonable price, near the beach ( it’s quiet, safety)
- Take speed boat back to Ha Tien.
- “Boat from Ha Tien to Mekong Delta. (Can anyone suggest best places to go on the Mekong Delta?) I was planning on spending 1 night on the Delta and then heading to HCMC straight after” .From Ha Tien is not boat went to the Mekong Delta, you should take the bus to Can Tho and spend one night there(at Can Tho you can visit: Ninh Kieu Wharf, Cai Rang floating market, My Khanh tourist village, etc) or is about to HCMC then start the tour Mekong Delta 2D1N.
Day 1: HCMC – Vinh Long – Can Tho
Day 2: Can Tho – Cai Rang Floating Market – HCMC
