I think Vietnam does not have online application for Visa. All are through services. However the fees are usually high.
No, as far as i know a lot of hotel can do visa online for you and travel agent as well

You do need to arrange your VN before you arrive at either a land border crossing or land at HAN or SGN. I used the 'visa on arrival' system on my '08 trip. I talk about it in my . It is cheaper to use the 'visa on arrival system' than buy a VN visa in your home country. If you are going to be in Cambodia before VN, then buy your VN visa in Phnom Penh or Sihanoukville. Cambodia is the cheapest, fastest place I know to arrange a VN visa. Good luck with your planning.
