As a US citizen you can stay in the whole Schengen area, of which Italy is a part, for 90 days without getting a visa. After these 90 days you have to leave Schengen for another 90 days before being allowed back for 90 more days.
If you would like to spend more time in Italy, you'll need a visa.
As a US citizen you can only stay in the Schengen area (with a tourist visa) for 90 days over a period of 180 days...
