HI,
I will be doing that in 2015 but not a train or plane though. By boat. This may give you an option of you have the time & cash.
Day 1: Siem Reap / Kampong Cham
Passengers will transfer by coach from the Victoria Angkor Resort & Spa to Kampong Cham. A packed lunch will be provided on the coach and we will stop at Kampong Thom along the way.
Day 2: Kampong Cham Area
Passengers will visit the hilltop temple of Wat Hanchey in the morning. In the afternoon, travel by bus to visit the Twin Holy mountains of Phnom Pros and Phnom Srey (Man and Woman Hill). Continuation to the ecotourism village of Choeungkok supported by the Frech-Cambodian NGO Amica.
Day 3: Tonle River / Kampong Chhnang
Excursion up the Tonle River to Kampong Chhnang by either coach or the ship depending on water levels. Beyond Kampong Chhnang travel by the Pandaw Explorer to discover the vast wetlands around the mouth of the river. Here many fishtraps may be seen.
Day 4: Phnom Penh
Passengers will visit Phnom Penh and will be provided with their own private cyclo*. The Cyclo Centre Phnom Penh which provides the cyclos is a charity that provides basic welfare and medical services to cyclo drivers .
In the afternoon, optional excursion (by coach) to the Killing Fields and the Khmer Rouge's grim Tuol Sleng or S21 detention centre.
Day 5: Cruising the Mekong / Border Crossing
We spend the day cruising in the main Mekong channel, an international shipping route for the Vietnamese border for the usual formalities.
Day 6: Chau Doc
Visit a Cham tribal village, a cat fish farm, and enjoy a trishaw ride round Chau Doc.
Day 7: Sa Dec / Cai Be
In the morning passengers will take an exciting Sampan boat excursion to Sa Dec via Vinh Long along canals and backwaters. Passengers will visit Sa Dec and see the local market and the ancient house of Mr, Huyn Thuy Le, the 'lover' of Marguerite Duras, a famous French novelist. At Cai Be, we will cruise through its colourful floating market which takes place daily in front of the catholic church.
Day 8: My Tho / Saigon
Early morning sail from Cai Be for My Tho, where passengers disembark in the early morning for a coach transfer to Ho Chi Minh city Hotel. Arrive at the hotel late morning.

There may be other flight options, but the above gives you a good idea.
There's no current passenger train service in Cambodia.
You could get there by bus, but it's a long journey. You'd be better off going from SR to Phnom Penh (PP). Having a night or two in PP, and then getting a bus from PP. Here's a useful link...

Steve how much does this trip cost? I actually planned to stay only a short period in Cambodia, but this sounds quite nice and I like the idea of going by boat (something I haven't considered so far).

You'd be totally okay posting that link in the open forums. We don't want links that are posted "for promotional purposes". You posting that link would be just helpful, and totally okay.
With new members, that determination is hard to make, and hard experience has taught us to err on the side of hardly allowing anything, but once we get to know members and trust that their purpose here is not to promote one company, there's absolutely no problem with links.

After looking at various options, I think your best bet is to take the bus down to Phnom Penh, $10. The boat is $35 and does not go everyday anymore. Not enough people will pay their overpriced fare. Flying this leg is over $150. Same using VN Air: REP-SGN. Advance purchase is $165.
PP has a few days worth of sightseeing, then decide if you want to pay $20 for the bus to HCMC or the boat via Chau Doc. Price is a bit more for the boat, figure $30-35. But, that only gets you to Chau Doc. You then need a bus to get to HCMC. More money, more time, more hassle to go via boat. I would take the bus. covers my bus trip between HCMC & PP.

You do need to buy your VN visa while in PP. All GHs will help you for a small fee. It is not worth your time/hassle to do it yourself IMO. If you do want some boat time, take a 1,2,3 day tour of the delta. Many travel shops offer delta tours. Get several quotes before deciding what company. Good luck with your planning.

I think I'll head to PP via bus and onwarts to Vietnam by boat (the cheaper variant, buses are actually not my cup of tea, espacially when covering long distances).
As I'm going to spend about 5 days in Bangkok before, I thought about getting the visa for Vietnam there.

I took a look at buying a VN visa in BKK. As I had heard before, it is more expensive than in Cambodia. might be helpful if you choose to buy it while in BKK. It will require 2 trips to VN Embassy and about $58 if you allow them 4 days to process. Fast costs more when dealing with the VN Embassy. Next day service is around $75. You can usually get a 30 day VN visa in PP for about $45 in 1,2 days.
I would read some blogs on the boat journey between PP & Chau Doc. Then on to HCMC. Know what you are dealing with. Good luck.
