Sorry but i have to tell you the truth that your itinerary will kill your time! so I have some following advices:
the 3rd Dec: you should go to My Tho ( Mekong Delta) to visit floating market and coconut candy makers... in early morning then after lunch, on the way to SGN, pass by Cu Chi Tunnels to discover underground world in the war. It will be a pity if you miss the Cu Chi Tunnels.
the 4th Dec: fly directly from SGN to Nha Trang early and enjoy 02 days in Nha Trang as your plan.
Then on 6th or 7th Dec, fly from Nha Trang to Da Nang and from Da Nang, take a train to Hue for enjoying a full-day city tour and overnight in Hue.Next day, take a flight from Hue to HaNoi.
( Hoi An is a must-visit place near Da Nang, if you have one more day, you can visit Hoi An first then back to Da Nang for a flight to Ha Noi)
However, If you wanna go to Phu Quoc, I advise you to change your itinerary up side down because Phu Quoc is belonged to the south of Vietnam. So the itinerary will be:
Ha Noi - Hue - Hoi An - Danang - Nha Trang - SGN - Cu Chi - My Tho - Phu Quoc - SGN for flight home.
Just make a careful decision then tell me, we will together clear and finalize your itinerary.

Thanks for the extensive advice...just wondering, do you you think I'd be missing out by not seeing Ha Long Bay? I've heard so much about it...
I wish I had more time to stay in Vietnam!
I think you do need to make time for HaLong Bay. The weather was terrible the day I went, but it was still worth the effort. It is one of the 'must see' when in VN.

You should spend 03 or 04 days in Ha Noi ( included Ha Long) as final destination of your Vietnam trip. It will more flexible for you. Ha Long is one of the most beautiful bay in The World and Vietnamese are always proud of it. So i think the reasonable itinerary in Ha Noi is:
- 01 day for Ha Noi half-day city tour combined visiting Bat Trang ceramic Villages. ( if you can spend one more day here, Duong Lam is the should-see ancient village of Ha Noi.)
- Cruise on Ha Long Bay: 03 days 02 nights on Board to enjoy the beauty of epic Ha Long. ( you can shorten this cruise to 02 days 01 night. It's ok too)

I agree you MUST include Halong Bay. From what I have read the train ride from Da Nang to Hue is one of the, if not the, most scenic train ride in VN. Cu Chi tunnels is not for everyone (reference blogs (incl pics) from various websites. Phu Quoc is a place (I believe) that if you spend the time travelling to and from then you really need to visit for 3+ days. Nha Trang - yes; Hoi An - yes if you can fit it in; Hue - day/o'nite trip.
