My experience has been that there are so few places where wheels are usable that they aren't worth the extra weight. Cobblestone streets, dirt paths and cracked sidewalks aren't friendly to wheels, not to mention what you might roll through in some places. My advice is to stick with a pack.

Since you suffer from back problem when backpacked for elongated hours, it is better to buy a backpack fitted with wheels. The wheels will aid in dragging the backpack while walking on a road in city areas.
I travelled 5 years ago without wheels and it killed my back. I'm off again in October for 5 months travel and have purchased a backpack with wheels. I got a Berghaus Wheeled Jalan, 2nd hand from ebay for £75. bargain
