Best would be who pays up if things go wrong. Don't just go for the lowest price but get one that covers what you want and what you might need.
You must tell them if you are indulging in sports and any activities even remotely dangerous because if you don't and you have an accident, they will not pay up (as those sorts of things make the policy cost more as they are "extras").
I've never bought insurance when travelling. So far, I've been lucky and haven't needed it (knock on wood!) But if you are going for a longer trip, or like extreme sports, it might be a good idea. I think my parents have used Blue Cross travel insurance, but they've been lucky too, and never needed it.
