Hey there.
Good way to do it really. take your time. 1 month is not a long time really when you consider travel times.

If you start in Spain and head to Italy, stop on your way in Carcassonne, France and maybe the south coast. Its on route and some great places to see.

Florence is worth a couple of days too and maybe see some other cities, Arezzo is a nother nice Tuscany town and less touristic than Florence. Venice is worth a trip too.

Never been to Rome. But Sicily is a wonderful destination as well. You can also fly direct from Spain to Italy on Ryanair. They offer deals from the South of Spain to Sicily for example.

Watch your step in Naples. If you find yourself there.

English might be tough in smaller towns. But bigger cities you will be fine. I didnt even hear any Italian in Florence anyway. Just English.

In Barcelona, be aware of ur belongings, mainly La Rambla street.

I go to bars alone when i travel all the time, not weird to me

I live in Rome, its quite a nice place, and I can advise on nightlife , florence is good if you want to go sightseeing, nightlife is very limited there. I reckon a few nights in Rome would be good, I have friends that work for a pub crawl group so they can show u all the bars and clubs and stuff. Madrid is defo worth visiting I just had a week long holiday there during February, beautiful place, highly recommend it. In terms of speaking only English, in Italy it shouldn't be too much of a problem just as long as you try to speak to the younger generation, and then in Rome I could always give you advice or help should you need any.
