In Madrid you should go to see Toledo. Nice place, you can get there by public bus. It takes about 40 minutes.
In Barcelona a must see is Montserrat. A monastery in mountains. Great views!
I don't know any tour companies, because I went there by myself.
You can check Viator. When I want a one-stop place to arrange activities and side trips, that's where I go. When I was in Barcelona last June, I spent a couple days in Sitges, which is only 20 mins south. It's mainly visited for its beaches so I'm not sure how great it would be in Sept/Oct.
In Barcelona itself the touristic things but worth it:
sagrada familia, parc guel (!!), Ramblas, walking in the hills up North, Casa Mila etc...
In the city there are a couple of companies that do roundtrips by bus. You can't miss them.

Hi! As someone recommended earlier, definitely try and see Toledo from Madrid - the journey is so short and the place is beautiful and quirky (not to mention there is a tonne of marzipan shops!). Also, I would say try and get away to Rascafria from Madrid. The journey takes around 2 hours or so and it takes you into the mountains where there are a couple of fresh water lakes. My advice here - do NOT go on a weekend!! It gets really busy as I found out the hard way... but it's still a great day out nonetheless
Montserrat.
This monastery is perched on a cliff and houses the Black Virgin. You can take a FGC train from Metro Espanya. Trains leave hourly and take you to the base of the mountain. From there you change either onto a cable car (the most fun) or a rack railway up the mountain. The ticket, that includes the metro to Plaça Espanya, the FGC train and the cable car cost about 20 euros and you can buy it at the FGC shop in the "El Triangle" shopping center in Plaça Catalunya or in Plaça Espanya.
Girona
A beautiful city with an interesting medieval Jewish quarter. Near is Figueras, where is the Dali Museum. You can take a RENFE train from Sants Station. www.renfe.es
Sitges
A colourful beach town popular among gays. You can take a train from Sants Station or Passeig de Gracia.
