Your main problem is insurance, as in if you insure a car in Italy and have a crash in Spain, they'll probably tell you to get lost.
I think Italy is like France in that if you but a second hand car from a dealer, they are required to give you a warranty if anything goes wrong, so second hand cars tend to be expensive. Unless you buy privately then the price is down to haggling.
Can you not travel between cities by bus, train or plane and then hire a car as needed? Parking can be a real problem in some towns.
