Three weeks in Malaysia and Singapore... that is a nice timeframe to explore the two countries fully but I recommend that the bulk of the time be spent in Malaysia as it has more to offer.
My suggestion is as follows:
To start with West Malaysia (9 days): Kuala Lumpur, Cameron Highlands, Ipoh (Gua Tempurung caves, Sam Poh Tong temple), GeorgeTown (heritage part, Penang Hill etc), Langkawi (cable car, sailing/ island hopping, beach etc) - then fly to Kota Kinabalu
East Malaysia: Trekking and hiking in Kota Kinabalu, Borneo for a week - return to Kuala Lumpur
Shop in KL (1 day), then travel south to Melaka (2 days) and then bus to Johor Bahru and Singapore city (2 days).
