Getting a visa in advance is definitely the best option nowadays. You can still do a visa run, but if you leave by a land border you only get 14 days when you re-enter. You still get 30 days when you enter by air, but leaving by air and coming back again isn't really viable for a visa run.
If you're going to Malaysia before you get to Thailand though, then you can get a Thai visa while you're in KL. I think it takes a few days though, so you'd have to factor a few days in KL into your travel plans.
I would imagine you can also get Thai visas in other Asian countries as well, but I'm not too genned up on this.
But if you're hoping to just go to an embassy while you're in Thailand and extend your visa then unfortunately this is only possible if you have a toursit visa before you get to Thailand.

You do not list a country on your profile page, but will guess the US or UK. A friend of mine is in Pattaya right now. He told me that Thailand has extended the free tourist visa program until March 2011. I have not received an answer from the Thai LA Consulate to confirm this. I would contact your Thai Embassy and ask about the free tourist visa program. You may get a free 60 day tourist visa from them. If they are not offering free visas, it will still be your best option to buy a tourist visa before leaving home. It is not cheap or practical to make a 'visa run' to Cambodia or Malaysia anymore.

I just heard back from the LA Consulate on the free visa program. It has been approved but not signed into law yet. Once signed, Thailand will again offer free tourist visas until March 1, 2011. Until then, you will have to pay for your tourist visa.
