For me, my best way of seeing that lot was on a coach tour many years ago. It seemed a bit expensive upfront but it was all the travel, all entrance fees, a guide, all accommodation and most meals. Also no trouble with luggage.
Middle as in June? It starts getting VERY busy in July. Even June you would need to book accommodation and probably travel beforehand. Also entrance to various sites online to save endless queues like the infamous Uffizi gallery queue. Everywhere will have queues, and often long ones.
Minimum I would say was four days in Rome, and a day in the others. That is full days. A visit to Pompeii too. There are a number of other places worth visiting too. I would like 3 weeks or so for such a trip.
Do be careful of pick pockets. If you suspect someone, just stare at them. Pick pockets hate that and will move on. The very young kids are the worst.
Florence and Rome - minimum 3 days each, more you do - more you earn.
Venice - from 1 day to... endless.
Pisa - 1 day.
